Basic Virtual Assessment

This 30 minute entry-level virtual assessment provides an expert overview of potential hazards in key areas of the home. Using a secure video call, an occupational therapist will guide you through a walk-through of your space, identifying fall risks, accessibility concerns, and simple modifications that can make daily living safer. You’ll receive verbal feedback and actionable suggestions you can implement right away.

Standard Whole-Home Virtual Assessment

This complete 60 minute home assessment provides a deeper look into every area of daily life—from entryways and bathrooms to kitchens and bedrooms. Conducted virtually by an occupational therapist, the session identifies barriers to safety, independence, and comfort. You’ll receive a written summary outlining specific recommendations, including layout adjustments, adaptive equipment, and home modification suggestions.

Enhanced In-Person Home Assessment & Follow Up

This premium service includes a detailed evaluation, functional assessment, and customized home safety plan. Beyond identifying environmental risks, we evaluate mobility, balance, and daily activity patterns to ensure your home supports your independence. You’ll receive a detailed written report, prioritized recommendations, and a follow-up consultation to track progress or discuss next steps.
